Admission Process
Raising Stars Autism Center is dedicated to making your admissions process as smooth as possible, which is why we have experienced administrative staff who will help you navigate through each step. We are also here to answer any questions you have about your health insurance and other benefits.

Six Steps to Enroll.
There are six steps to the application process.

  1. Completing Documents: Submitting your information to our experienced administrative staff is the quickest and most effective way of acquiring information or services, benefit and beginning therapy. Application will be available at the office upon request.
  2. Provide Document:
    • Insurance Card
    • Insurance Card
    • Diagnostic Report
    • Prescription
    • Comprehensive multi-disciplinary evaluation (CMDE)
  3. Intake: Our administrative team will verify your Insurance coverage and contact you to let you know about your treatment options.
  4. Assessment: Our administrative team will work with you to schedule your child’s initial evaluation. A comprehensive intake packet will need to be completed and returned to our administrative team prior to your assessment time.
  5. Discuss a plan: You and your child will meet with our Qualified supervising Professional (QSP) to complete the initial evaluation and to collaborate with you on a treatment plan and schedule.
  6. Start Services: Our office manager will contact you to let you know your child’s services have been authorized by the insurance and to set your start date. You will be also be instructed to complete the Welcome Packet which contains important information pertaining to your child’s safety and rights while receiving services.
